By using our services, you agree to receive care via telehealth, which includes communication through secure electronic means. Telehealth allows for remote evaluation and treatment of your health concerns. You understand and agree to the following:

• You must be located in Arizona, Florida, or Maryland at the time of service.
   
• You have the right to ask questions, decline services, or stop care at any time.
   
• Virtual care has limitations, including the lack of a physical exam.
   
• You may be referred to in-person care when appropriate.
   
• A detailed telehealth consent form will be reviewed during your intake process.
